A single wrist-mounted IMU measuring movement smoothness (SPARC) during active abduction was significantly lower in the rotator cuff disorder group; flexion showed a numerically lower but non-significant difference. Diagnostic accuracy for distinguishing groups was modest (AUC 0.70-0.74, specificity 0.78-0.89, Kappa 0.32-0.38), and pain scores (NRS) did not differ between groups.

Granskningen

With n=30 split across two diagnostic groups, this is a hypothesis-generating pilot, not a validated diagnostic tool, no power calculation is reported and none of the reported CIs are given, so the AUC/specificity point estimates are imprecise. Kappa of 0.32-0.38 indicates only fair agreement, which undercuts the headline AUC figures; the reference standard is PT clinical classification rather than imaging or a structured gold-standard exam, raising circularity/incorporation bias concerns. No mention of blinding between the person classifying disorders and the person analyzing IMU data, and no external validation cohort.

Begränsningen

The single biggest issue is sample size and lack of an independent validation cohort, before this tool could be trusted clinically you'd need a larger, prospectively powered sample with a blinded, imaging-confirmed reference standard and external replication.

Referensstudie

This extends the general movement-smoothness/SPARC literature (originally developed in stroke motor recovery research) into shoulder pathology, and sits within a growing wearable-IMU diagnostics trend rather than building on one specific shoulder landmark trial; I'm not aware of a single foundational paper it directly contradicts or confirms.

No, this should not change Monday's practice. It's an interesting proof-of-concept for low-cost objective shoulder assessment (useful for future telerehab tools) but the diagnostic accuracy and agreement statistics are too weak and unvalidated to substitute for clinical exam or imaging.
